The Connecticut Hospital Association (CHA) in collaboration with the Connecticut Behavioral Health Partnership (CTBHP), and Carelon Behavioral Health (Carelon), cordially invite you to participate in a statewide Community Care Team (CCT) Collaborative.
Join us to:
- Network with key stakeholders from across the state
- Celebrate successes of the CCT model
- Learn about legislative updates
Participants will have the opportunity to learn from established teams, engage in open dialogue, and explore strategies to maximize the impact of CCT’s for the individuals they serve.
