Community Service Award

The Connecticut Hospital Association (CHA) and the Connecticut Department of Public Health (DPH) invite CHA members to submit entries to receive the 2027 Connecticut’s Hospital Community Service Award. The award recognizes a Connecticut hospital or health system that has made an outstanding contribution to its community. Entries will be judged by a panel comprising representatives from Connecticut acute care hospitals, a DPH representative, and a member of the Connecticut media.

Each entry must comprise the following:

• The name, title, address, telephone number, and email address of the project’s contact person
• A one-line description of the program or service
• A description, in 500 words or fewer, outlining the program or service using the five criteria below to organize your entry

Criteria for the Award

1.  A hospital or health system must have developed and implemented an innovative program or service that meets a specific need, or addresses concerns of a targeted segment of its community.
2. The program must be established and under way for a minimum of one year. Please specify the start date, and whether the program is ongoing or has a determined end date.
3. A hospital or health system should have invested both time and resources in the development and implementation of the project. Please specify funding sources, including dollar amount, in addition to the number of employees or volunteers dedicated to the effort.
4. The project should be an extension of the usual role of an acute care hospital/health system and must be public health oriented. Please indicate the number of people or percentage of your service area that have benefited from the project. Also, indicate if services were delivered for free or at a cost to participants.
5. The program should be able to be feasibly replicated, serving as a model for other hospitals. Explain how.

Award Entry Guidelines

Each hospital and/or health system is eligible to submit multiple entries, and joint entries are also encouraged. To submit multiple entries, you must refresh the application webpage after submitting an entry.

The total word count of your entire submission should not exceed 500 words, not including contact information and the one-line program description.

Note: There is a two-year period of ineligibility for previous Community Service Award winners.

The award entry deadline is Wednesday, January 13, 2027.
