High Reliability is the study of human performance in complex systems and includes systems thinking, event analysis, techniques to minimize human error, approaches to improve processes, and tactics to move organizations to a culture where a safety-first focus drives higher levels of performance across all domains. To deal with challenging work conditions, high-risk operations and mitigate threats to success, HROs create organizational mindfulness. Mindfulness is a rich awareness and a capacity for action that jointly facilitates a capability to discover and manage unexpected events before they escalate into crises and catastrophes. HRO leaders build situational awareness and organizational mindfulness to anticipate problems and events, including the integration of huddles, rapid escalation protocols, measurement dashboards and control loops to help leaders build system resilience to respond to the dynamic, ever changing production pressures involved with healthcare delivery while ensure safe, high-quality, patient-centered outcomes.
