CHA Presents on HRO Forward at IHI Patient Safety Congress 2025


This month, the Connecticut Hospital Association (CHA) convened with healthcare leaders from across the country at the Institute for Healthcare Improvement (IHI) Patient Safety Congress 2025 in San Diego, California to present on CHA’s statewide initiative to advance patient safety utilizing high reliability practices.

CHA Vice President of Quality and Patient Safety Alison Vail and Director of Clinical Excellence and Care Redesign Ellen Crowe led a workshop on HRO Forward and Connecticut hospitals’ and health systems’ journey to zero preventable harm.

CHA launched HRO Forward in the fall of 2023, building on work that began when the first high reliability initiative launched in 2012.  Vail and Crowe outlined the steps of the HRO Forward initiative — starting with Phase I, a standardized high reliability maturity assessment using the “IHI Safer Together: A National Action Plan” tool to identify areas of improvement and develop a roadmap to accomplish statewide collaboration goals.  Now, in Phase II, hospital participants are working together to develop and implement the strategies identified in the roadmap to advance the delivery of high reliability care in the state.  Healthcare professionals across disciplines are engaging in extensive training to learn about the latest innovations in safety science and implementing collaborative strategies to continue elevating the quality and safety of care in Connecticut healthcare settings.

The IHI Patient Safety Congress brings together healthcare leaders, professionals, and learners from across the full spectrum of patient care, all with one shared mission of improving the safety and equity of healthcare.  The National Action Plan to Advance Patient Safety, an initiative designed to enable a total systems approach to safety and reduced harm across the continuum of care, was the cornerstone of the congress.
