The Connecticut Hospital Association (CHA), in collaboration with the Connecticut Behavioral Health Partnership (CTBHP) — which includes the Connecticut Departments of Mental Health and Addiction Services (DMHAS), Children and Families (DCF), and Social Services (DSS) — and Carelon Behavioral Health (Carelon), convened community care teams from across the state at an in-person statewide Community Care Team (CCT) Collaborative meeting on Wednesday, June 25.
The event brought together stakeholders involved in running CCTs to celebrate the successes of the CCT model, network with one another, and learn about key legislative updates and new state laws passed during the 2025 legislative session.
The event included presentations from established teams and fostered an open dialogue among participants to explore strategies to maximize the impact of CCTs for the individuals they serve and tackle ongoing and new challenges. Topics included discussion on housing and homelessness, building community relationships and resources, probate/conservatorship, addressing co-occurring and co-morbid health conditions, and managing burnout and capacity in care teams.
Nearly 60 participants joined the meeting. There are currently 18 CCTs operating in several regions around Connecticut.
