The Community Health and Wellness Day is a combined clinic and health fair to provide access to services and community resources. It bring health screenings, clinical exams, and health education to adults with limited or no health insurance in the Middletown area. Available clinical services include: general medicine exams, prostate cancer screening, clinical breast exams and access to mammograms, access to colorectal cancer screening, skin cancer screening, access to lung cancer screening, dental cleanings, blood pressure screening, foot screenings, HIV testing, hepatitis C testing, and nutrition education. |

The Greater Middletown Health Enhancement Community Coalition (GMHEC Coalition) is a place-based initiative that supports long-term, collaborative, and cross-sector efforts to improve community health and well-being through broad, systemic change. The goal is to build healthy and equitable communities that actively work together to prevent the development of disease. Through this work, the GMHEC Coalition seeks to engage in initiatives that improve the social, economic, and physical conditions within communities that enable individuals and families to meet basic needs, achieve their health and well-being goals, and thrive throughout their lives. Middlesex Health staff participate in and provide administrative support for this coalition. |

Greenlight Manhood Planning Committee |
The goal of Greenlight Manhood is to create trusted and safe spaces with resource access for men of all backgrounds, prioritizing men of color, to engage in peer-to-peer supported conversations and activities about mental health and its impact to total health and well-being. The roundtables and summits focus on conversations about mental well-being, proactive lifestyles, empowering men to give themselves the greenlight to exist fully in their manhood. Middlesex County NAACP Health Committee |
The mission of the Middlesex County NAACP Health Committee is to promote, protect and maintain the health and well-being of African Americans, Latinos and all people of color in Middlesex County.; this will be accomplished by assessing health needs and advocating for health equity in order to achieve positive health outcomes.The Latine Focus Group Project is the Health Committee’s most recent accomplishment. This project is the product of a partnership between the Middlesex County NAACP, Middlesex Health, and and community partners and members these entities serve. The overarching goals of the project were to better understand community-identified social determinants of health and other factors that negatively impact health outcomes for people of color; to strengthen the connection between people of color, healthcare organizations and community service organizations; and to support and create changes based on community-identified solutions in order to advance health equity. The 2019 portion of the project focused specifically on Black/African American residents of Middlesex County. This portion of the project focused specifically on Latino/a/e residents of Middlesex County. |

For those who screen positive, a referral is made to inpatient social work (for hospitalized patients). The event may include but not limited to the following: medical exams by physicians, foot washing and podiatry, behavioral health, prescriptions, smoking cessation, child care, point of care testing, nutrition counseling, vaccinations/flu shots, distribution of gently used coats, new shoes and reader eyeglasses. |

Neighborhood Health initiative is designed to bring more health prevention services and support directly to the people and communities we serve. It’s part of Hartford HealthCare’s larger commitment to helping all people live their healthiest lives by providing more outreach and healthcare opportunities throughout Connecticut. Neighborhood Health was developed in collaboration with trusted community partners throughout the state. These groups and individuals helped determine the health needs and priorities for their residents and communities. These innovative health clinics will be adaptable, flexible, and open to feedback to ensure access to needed services and programs. Mobile “CareVans” visit and operate daytime health clinics at specifically chosen locations every week. They offer a variety of health services including screenings, mental health counseling, medical referrals, education and support. The Pfizer 2023-2024 COVID-19 vaccine and several other non COVID immunizations are also available. |
