Veronica Mansfield, DNP, APRN, AE-C, Medical Director for the Middlesex Health (MH) Center for Chronic Care Management, is a certified Asthma educator with a special interest in pediatrics. In 2016 she led MH in its participation in the New England Asthma Innovation Collaborative (NEAIC), which partnered Connecticut, Massachusetts, Rhode Island, and Vermont to reduce ED visits and hospitalizations for children with persistent Asthma, resulting in statistically significant improvement in reduction in need for quick relief inhalers, an increase in Asthma action plans, and reduced missed work and school days.

In 2022, Asthma was identified via Middlesex Health’s Community Health Needs Assessment as one of the top 10 health issues in our community.

In 2023, based on Veronica’s leadership with NEAIC, the Department of Public Health (DPH) reached out to partner MH with the state’s home visiting program, Putting on AIRS (POA). POA uses home visits to reduce acute asthma episodes and improve control. Middlesex Health was the first health system to partner with DPH to provide this comprehensive program under Veronica’s tutelage.

The POA’s interventions provide a thorough evaluation of participants’ asthma symptoms, individualized asthma education, an environmental asthma triggers assessment, the identification and the elimination, or reduction, and implementation of trigger reduction strategies in the home environment. The program is directed by Veronica Mansfield, and it is her passion to improve the lives of those suffering from this chronic disease. Veronica personally sees all patients for their initial assessments as an APRN and then connects the patients with our team, which includes registered nurses, community health workers, and a population health coordinator. Veronica’s connections and known expertise in our community facilitated an expedited rollout of the program.
